One person was killed, and three were injured in a water cannon accident aboard an offshore support vessel last weekend in Brazil.

Brazilian media have reported that the incident happened aboard the Astromaritima Navigation’s Astro Tamoio OSV during a firefighting exercise on Saturday, offshore Brazil.

According to reports from Brazilian media, Eloa Bernardo dos Santos, 23, was fatally injured on Saturday afternoon and succumbed to the injuries on Monday evening in Santa Maria Magdalena Hospital in Rio de Janeiro.

The O Globo website said that the water cannon malfunctioned and turned from the sea towards the deck. The crew attempted to intervene and fix the malfunction but because the deck was not emptied at the time four crew members were hit by a strong water jet.

Firefighting tests have to be conducted with the oversight of the Navy which was not the case during Saturday’s test.

O Globo said that Astromaritima, a provider of services to Petrobras, made a statement on Monday evening claiming that the test was conducted to check if there was any leaking from the water system. The company said that such a test does not require the Navy to be notified.

The vessel where the accident occurred, the Astro Tamoio, is a 2016-built, 100 meters long offshore support vessel which can accommodate 22 crew members.
